In addition, the licensee provided inaccurate information to the department pertaining to how a child in the care of the center on 1/26/21 arrived home. The licensee reported she transported the child home; the child's mother reported the child was picked up by his father after confirming the information with the father.

Description: The licensee failed to report to the department a change in room usage, such as using rooms not previously approved for use at least 20 working days prior to the change when her bedroom was utilized for the child care on 1/26/21. The licensee's bedroom has never been approved for use by the childcare. Changes in room usage shall be approved by the department prior to the change.

Description: The licensee failed to report to the department any construction or remodeling on the premises that has the potential to affect an area accessible to children or a condition of the license. Notification shall be provided in writing before the construction or remodeling begins. During the visit the licensee advised the department that at the end of December 2020 or early January 2021 her living room floors were sanded and refinished. The living room is the main daycare area.
2021-02-03 250.04(3)(g)1. Report - Law Enforcement Contact - Harm
Description: The licensee failed to report to the department an incident involving law enforcement within 24 hours after the occurrence when she learned on 1/26/21 that "911' was summoned by a household member/child care employee for a household member/child enrolled in the childcare resulting in the Milwaukee Police Department responding to the child care center. This licensee failed to report this to the department until 1/30/21.
