Noncompliance Area: It was observed that the kitchen area was left accessible to children and contained sharp knives and the stove was one while no staff were present in that space.

It was observed in the preschool outdoor play space that the black mat on the playground equipment had separated from the landing and curled causing a tripping hazard and the shed, used by children, contained sharp edges around the flashing on the lower part of the shed door.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: It was observed that three emergency contact forms did not accompany the children in the space where they were receiving care.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: It was observed that the emergency transportation plan was not contained in the facility van and did not accompany staff persons who leave on an excursion with the children.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: It was observed that child #1 had medication being stored at facility without signed parental consent for administration.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Noncompliance Area: It was observed in the Kindergarten bathroom that the baseboard was peeling away from the wall. It was observed in the outdoor shed that a broken window in the rear of the shed was a visible hazard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: It was observed in the school age room a large purple bin was cracked and broken causing a pinching hazard. In the infant play space a blue cushion seat was ripped and not in good repair exposing foam, in the young and older toddler play spaces the same red wedge seats in both room areas had corners that were ripped and not in good repair exposing foam.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: It was observed in the older toddler room there were plastic bags hanging from their cubbie area leaving them accessible to mouthing children.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: It was observed that the fridge temperature in the infant room measured at 48 degrees Fahrenheit.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: Parents of child #1, #2, #3, #4, #5, #6, #7, and #8 did not receive the original agreements.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement.

Noncompliance Area: Child #6 had a current health report on file dated 2/13/18 but previous health report on file was dated 10/27/16. Child #6 did not have a health report updated within a 12 month period as required.

Noncompliance Area: It was observed that the safe routes, also known as pick up and drop off procedures, established by facility were not posted at a conspicuous location in the child care facility.

Following review, a parent shall attest to the accuracy of information in the emergency contact information and the financial agreement at each review by affixing a dated signature to the record.

Noncompliance Area: Staff person #1 had transcripts on file verifying degree level, however, no certificate or school name on transcript was found in file. File does not contain sufficient verification of degree level by an accredited college or university program.

Correction Required: A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: It was observed that the instructions for contacting the appropriate regional day care office was not posted at a conspicuous location at facility.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.

Noncompliance Area: Facility's emergency plan training was last completed for all staff on 9/1/17 and has not been updated; therefore emergency plan training has not been completed annually as required.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: It was observed that the water temperature in the 4-year-old room bathroom measured at 118 degrees Fahrenheit.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Correction Required: A first-aid kit shall be in a child care space. When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.
