Description: Building Surfaces

Noncompliance Area: Dust and a few small dead bugs were observed on windowsills in the Older Toddler and Pre-K 2 classrooms. Corrected on site by wiping down the windowsills. Dirt was observed in the side seam of the changing table pad in the Young Toddler room. Corrected on site by wiping the seam of the pad on all 4 sides.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: A rocking chair was positioned in front of the door to the playground in the Blossoming minds classroom, partially blocking the exit. Corrected on site by moving the chair to a different area of the room.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.

Noncompliance Area: The file for Staff Person #1 contained a TB test result that was dated 7/5/24, which was after the staff person started working at the facility (see code sheet). The TB test was not done and documented within 12 months prior to providing initial service in the childcare center.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: A sensory table on the School Age Playground, was observed to have a lid that was peeling. The table had sand that was full of mold, which was accessible to children.

Correction Required: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. Noncompliance Area: In the school aged room, one of the stools was observed to have loose screws in the leg, causing the stool to wobble and be unsafe for children to sit on.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Building Surfaces

Noncompliance Area: In the infant room, a sewage pipe burst, contaminating the floor in the closet. The floor was observed to be covered with plastic bags and was observed to be damp underneath with an odor coming from the closet. Staff confirmed that sewage was tracked through the infant room on the carpet. Carpet was cleaned by the director but was not professionally cleaned. Carpet that was cleaned is a space where infants crawl on the floor.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. DIRECTED PLAN OF CORRECTION: Carpet in the infant room must be professionally cleaned by a carpet cleaning service. Director must provide documentation from cleaning company that the cleaning was completed.

Noncompliance Area: In the toddler room, 1 staff was observed to be caring for 5 children ranging from an infant to older toddlers.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.
