After review of the process with licensing, the center has updated the six-month review procedure to ensure that the Emergency Contact Form and Financial Agreement currently on file are attached directly to the parent review packet along with the new Six- Month Record Review & Attestation form. Parents will review the attached records and sign the attestation confirming that all information is accurate, complete, and up to date. If any changes are needed, updated forms will immediately be completed and maintained in the child's file. The updated process has been implemented. |
