LHNS has awareness to every 5 years and was in process of all above. We missed "exact date vs every 5 years." All above were in process with appts. All employee files will now record the month and date in the month, also with the year (as we previously followed), to ensure not one day passes the "60 month" of clearances and mandated reporter (not " 5 years" . This will include FBI DHS prints, state police, child abuse , NSOR and the mandated reporter training. All employees will complete prior to contact with children for renewals and new hires, to comply with CPSL and Chapter 3490. |

Staff Person #1 had the PAC review the exam, complete the missing examination for communicable diseases and record the results for that exam. The medical examiner (PAC) had to re-sign the health assessment at the missing item, re-stamp the office as proof of location, and redate the correction on the health assessment for Staff Person #1. This occurred on the same day of the inspection, 10/23/24, and was submitted to the DHS representative as proof of completion. |
