Violation Observed: On the day of the inspection observation indicated that children were being walked across an active driveway without safety systems in place. There is a very active car wash behind the building, and the exit is the driveway in which the center uses to walk back to their approved playground and to cross when going for a walk. Center had previously submitted a safety procedure plan for walking back to their playground but it was not followed on the day of the inspection. Interviews with the sponsor indicated that the staff are required to wear safety vests and use a stop sign, but those items were not available nor accessible at the time of the inspection. Ensure that a safety procedure is formulated and followed, and submitted in writing to OOL.

The center shall ensure that all staff members are trained in the method of keeping track of children and know how many children are in their care at all times.

Violation Observed: Ensure that all staff have completed training pertaining to the center's policies and procedures for tracking children and are aware of the number of children in their care at all times.

Violation Observed: Ensure that all staff are retrained on correctly tracking children. On the day of the inspection staff in the infant room were asked how many children they had and they did not have the correct number. Submit retraining document with staff signatures to OOL

Violation Observed: Ensure that the center washes and disinfects toys mouthed by infants and toddlers before being given to another child. On the day of the inspection staff were observed removing toys from a toddlers mouth and placing on the floor back into the environment without washing and disinfecting.

Violation Observed: Per the NJ Department of Health COVID-19 Public Health Recommendations pursuant to Executive Order No. 149, ensure that all health and safety guidelines are adhered to as required. Ensure all staff that are preparing and/or serving food/bottles wear gloves. On the day pf the inspection staff were observed not wearing gloves while assisting children during lunch.

Violation Observed: Ensure that the ripped diapering mat in the bathroom in the rear of the building is replaced.

Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. On the day of the inspection the staff person in the infant room walked out of the classroom numerous times and left the children in the classroom. When she leaves the classroom to change a child's diaper she does keep the remaining children within her line of site. This staff person should call for assistance in order to maintain proper supervision.

Violation Observed: Per the NJ Department of Health COVID-19 Public Health Recommendations pursuant to Executive Order No. 149, ensure that all health and safety guidelines are adhered to as required. Ensure all COVID positive cases are reported to local health officers. Ensure that all close contacts are required to quarantine for the time which is directed by the CDC. Center had a positive case in which close contacts were allowed to test the day after being notified of a positive case and return to school with a negative test. Children would have needed to be out of school for a minimum of 7 days only to return on the 8th day after testing negative on days 5-7. Center to retain a copy of the negative test results.

Violation Observed: Per the Updated COVID-19 Standards- Child Care Centers shall ensure that staff wear masks appropriately. On the day of the inspection a staff person in room 2 was wearing her mask below her nose because the masks does not fit properly. The mask falls when she speaks. Ensure that masks wore fit and are worn properly.
