Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 08/19/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the facility at the time of the inspection. Please submit the Plan of Corrections in the LMS Portal within 10 days of receipt of the Statement of Deficiencies. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Criminal History Affidavit’s will be complete *New Employee Training will be documented with a complete date and signature *Annual training will be 24 hours *Emergency drills will be documented *New employee training topics have been updated

Initial Comments: There were no deficiencies observed during the Initial Compliance Inspection conducted on 6/25/2025. A paper copy of the Notice of Inspection Rights was given to the Licensee at the beginning of the inspection. During the exit interview the following items were discussed but are not limited to: *Ensure all new staff have a complete file per R9-5-402 *Ensure outdoor equipments remains free from bird feces *Traffic cones or another physical indicator will be placed behind the swings to indicate the boundary of the Pre-K playground. *Update the Director in the Licensing Management System Portal *Blinds cords must be inaccessible to children *Staff attendance records *Monthly fire drills *Emergency, Information, and Immunization Record cards *Children's sign-in and out sheets *Classroom rosters *Child abuse and neglect *Accessible drinking water in activity areas *Written approval for personal products supplied by the facility *Labeling of personal products *Specific food items on the menus (ie mixed vegetables, tropical fruit) *Discipline and guidance *Notifying parents and documenting injuries *Illness and infestation log *Medication storage and forms *Field trip form *Notify the Department of closure for any days. *Keep documentation for 12 months. *Mops, push vacuums, and brooms must be inaccessible to children. *All toxins and chemicals must be in accessible to children.
