|Findings: Inhaler for TK was at center with no medication forms and asthma action plan.|

|Findings: Freezer had substance all over bottom and on food items in the freezer.|
|Findings: 38 students in care with 3 teachers in childcare room. Children then moved to the gym, but only 2 teachers went over to supervise 38 students.|
|Findings: Under sink in dedicated childcare area cleaning products were found under the hand washing sink.|
|Findings: The emergency and disaster plan was not practiced as required at all of the specified intervals.|
|Findings: Potentially harmful items were stored in a location accessible to children.|
|Findings: Potentially hazardous food was kept at a temperature above 40 F.|
