At 7:43 AM, the 3s and older classroom had 21 children with only one staff, four children @ age 3. At 8:53 AM, the infant room had six infants present with only one staff. A staff working with children and employed since 2-25-25 does not have a completed background check or a background check status of in process. Last documented smoke detectors and carbon monoxide check was documented on the monitoring summary 10-30-24. Fire, Tornado drills were last documented on 10-30-25 monitoring visit. |

Plan: At time of visit school was understaffed due to staff calling out for the day and not following call in policy of two hours prior to shift speaking with a member of management to verify shift could be covered before not coming in for the day. Meeting was held with all employees to discuss this policy and importance of following so this does not occur again.
