Findings: In reviewing the emergency cards for each enrolled child, it was discovered that SS's emergency card does not contain her physician's phone number. CS's emergency card does not contain any physician information. Please have the parent (same) modify the emergency cards, adding the needed information and initial and date the card for the update that occurs when making the correction. Provide proof of this added information to OCC for the correction of this non-compliance.

Findings: In reviewing children's records, it was revealed that four year old, SB, three year old CS and two year old, SS, do not have any evidence of having had any blood lead testing. Child MS is four years old and only has evidence of having one blood lead test at 12 months old. Each child in care should have blood lead testing at 12 months and again at 24 months. Please notify the respective parents so that they can schedule the blood lead test and provide you with results to keep in their files. It was also discovered that CS's immunizations are not up to date, but her parent has signed the religious exemption. Child JB needs a MMR and a varicella immunization according to her age. Child IS needs another DTP, Polio, HIB, Hep B and Prevnar vaccination according to her age and the vaccination requirements for children enrolled in a Maryland child care program. Please notify the respective parents so that they may obtain the necessary vaccinations and provide you with an updated immunization record for their file. Please submit these documents to OCC for the correction of this non-compliance.
