Noncompliance Area: Children were observed sleeping in pack-and-plays that did not have at least 2 feet of space on three sides of the rest equipment.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: According to the log on file at the time of inspection, fire drills were conducted on 6/15/25 and subsequently more than 60 days later on 8/17/25.

Noncompliance Area: According to the log on file at the time of inspection, manual tests of the fire detection system took place on 7/8/25 and subsequently more than 30 day later on 8/17/25.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. §  1016(c)). To verify operability, a child care facility shall manually test all fire detection devices or systems at least once every thirty days and shall maintain a written record of the testing with the facility's fire drill logs. Description: 12 months prior to service and every 24 months thereafter

Noncompliance Area: At the time of the renewal inspection follow up visit to verify corrections, the plan of corrections for the violation citing 3280.151 on SIN-00270043 had not been fully implemented. On 8/19/25, the most recent health assessment on file for staff person #2 was dated in September of 2022.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Description: Supervised at all times

Noncompliance Area: At the beginning of the inspection, staff person 1 was the sole caregiver present. Staff person #1 could not see the two preschoolers from where she was in the next room. Staff person #1 then walked into the kitchen to make a phone call, leaving all 5 children unsupervised. Later on in the morning, facility person #2 (volunteer) was observed taking a child to use the bathroom in the basement. The child was not supervised by a staff person during this time.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Regulation: 3280.123(a)(3)

Description: Services proceeded

Noncompliance Area: Child service reports were not completed timely during the previous year for children #1, #2, #3, and #5. Child #1 - The child has been enrolled for more than 12 months. The record did not include any Child Service Reports. Child #2 - The most recent Child Service Report was from November of 2024. Child #3 - The child enrolled in October of 2024. The first Child Service Report was completed more than 6 months later in May of 2025. Child #5 - Child Service reports were completed in August of 2024 and subsequently more than 6 months later in April of 2025.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3280.124(f).
