Noncompliance Area: On 9/15/20, it was observed that the climbing mats in the Ones classroom were torn exposing the stuffing. CORRECTED ON SITE.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: On 9/15/20, it was observed that child 5 did not have signed parental consent for emergency care on file.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: On 9/15/20, it was observed that child 1 did not have addresses on file for their release persons.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: On 9/15/20 it was observed that an outlet cover was missing in the hallway outside of the Pre K counts classroom. CORRECTED ON SITE.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: On 9/15/20, it was observed that rubber gloves were missing from the first aid kit in the Kindergarten classroom. CORRECTED ON SITE.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: Small pebbles less than 1 inch accessible on the infant/toddler playground.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care. Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: During naptime observation on 7/19/19, a child was unsupervised at the front of the Kinder Ready room. Staff's placement did allow proper supervision of this child--child was unable to be seen by either staff in the room.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Supervision is to see hear direct and assess at all times including naptime.

Noncompliance Area: Child 5 did not have name/address/phone of designated release persons attached as noted on emergency contact form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Emergency contact forms were not updated in a 6mth period in the Yg Tod and Older Tod rooms.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Child 6 and 7 did not have medication logs completed and signed by parent for administration of medication.

Correction Required: A parent shall provide written consent for administration of medication or a special diet. An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: Bottles, cups, and breast milk were not labeled for individual use in the infant room.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Child 3, 4 did not have emergency contact form and agreement updated in a 6 mth period. Child 5 did not have emergency contact form updated in a 6mth period.

Correction Required: Information in a child's record shall be kept current by the operator.A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Furniture was stacked on tables during naptime posing a hazard to children placed directly beside the equipment.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Exposed tree roots pose a trip hazard on the infant/toddler playground.

Correction Required: Outdoor or indoor play space shall be safe for large muscle activity.

Noncompliance Area: Toxic tomato plants located on the inf/tod playground.

Correction Required: Toxic plants are not permitted in a child care space.
