Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. On 10/17/23 two children were not signed out, on 10/24/23 one child was not signed out, and on 11/6/23 one child was not signed out.

Equipment and furnishings were not sturdy, stable and free of hazards. A clear bucket with a blue handle, accessible to children in the sandbox on the playground, was observed with a 4" crack on the bottom and a 4" crack on the side containing sharp edges and pinch points.

An incident report on file for a child enrolled on 8/30/23 was not completed as required. The report did not indicate the type of injury the child had.

The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. Per 15A NCAC 18A .2817 TOILETS (a) In child care centers, toilet tissue shall be provided in each toilet room and stored in a clean, dry place. The toilet room shall include or be adjacent to a handwash lavatory. Storage in toilet rooms shall be limited to toileting and diapering supplies. All toilet fixtures shall be easily cleanable, and in good repair. Toilet fixtures shall be child-sized, adapted adult toilets or potty chairs. A sand/water table and materials used for sand/water play were observed in the bathroom used by children enrolled. Per 15A NCAC 18A .2820 STORAGE (c) Non-aerosol sanitizing, disinfecting, and detergent solutions, hand sanitizers, and hand lotions shall be kept out of reach of children when not in use but are not required to be in locked storage. These solutions shall be labeled as sanitizing, disinfecting, or detergent (soapy water) solutions. Hand soap other than that which is in bulk containers is not required to be kept out of reach of children or in locked storage. Disinfectant wipes were observed on the child-sized sand/water table stored in the bathroom used by children enrolled.
