Staff Person #3 observed Child #1 standing outside the window of the DPS A classroom. When Staff Person #3 realized Child #1 was alone on the playground they notified Staff Person #4 and #5. Staff Person #4 and #5 immediately retrieved Child #1 from outside on the playground and returned Child #1 to the preschool room. Staff Person #1 and #2 did not know the whereabouts of Child #1 for approximately 5 minutes.

Staff members attended supervision training on 8/25/22, during this training, staff members learned where to position themselves to ensure that all children are supervised at all times on and off facility premises, especially during transitions. As of 8/29/22 all transitions will be supervised by a member of management to ensure that children are supervised at all times on and off the premises, management will also ensure that the staff members have all the tools they need to complete a safe transition, making sure that a staff member is in front of the line and another in the back of the line to make sure no children elope from the group.
