Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: It was observed that there was peeling paint in multiple child care spaces: in the 2 year old room, about 5-6 areas of peeling paint on the wall to the right, under the bulletin board. Also, under the window, 5 spots of peeling paint. To the right of the window, under the bulletin board, 4 spots of peeling paint, and the window sill edge has multiple areas of peeling paint. In the 1 yr old room, about 5, 1-2 inch areas of peeling paint observed on the right wall (under the bulletin board, near the diaper changing area). On the right wall, under the bulletin board closer to back of room, there are 4 areas of approximately 2 inch spots of peeling paint. On the far wall, under the coat rack, 3 areas of 4 inch spots of peeling paint were observed. Also on the far wall, to the right of the mirror, observed 5, 2-3 inch spots of peeling paint. In the Preschool room, on the wall with the door, it was observed that there was 10, 1-2 inch areas of peeling paint. On the right wall, toward back of room, about 1 foot up from the floor, 5 , 1-2 inch areas of peeling paint observed. On the back wall, in the right corner, about 1 foot up from the floor, there was a 3-4 inch area of peeling paint. On the back wall, in the calm down area, there were 3, 1-2 inch areas of peeling paint. On the wall to the left of the back door, from 6 inches to 2 feet up from the floor, there were approximately 20, 1/2 inch-3 inch areas of peeling paint. The natural wooden shelf with a black wood surface (left wall) had peeling paint on all edges and multiple areas of peeling paint on the top. The right wall, under big calendar bulletin board, about 6 inches up from floor, had two 4 inch areas of peeling paint.

Noncompliance Area: On 6-7-2022 Staff #1 tucked child #1's blanket under the four posts of his cot while he was napping. The child's arms and legs were under the blanket. The child was restrained by the blanket and was not able to get out without help from staff.

Correction Required: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. The required staff:child ratios must be maintained at all times.
