Noncompliance Area: The program reported that a child was left unsupervised on the playground for approximately 2 minutes.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. 3. The legal entity will ensure that name-to-face checks are conducted during transitions are supervised for the toddler classroom for a period of one month, beginning at the time of approval of this plan. To show compliance with this correction, the program will submit documentation of each check on a form approved by DHS on a weekly basis.

Noncompliance Area: Evidence that the program contacted the appropriate regional office within 24 hours of a child receiving emergency care was not available. A written report submitted to the appropriate regional office within 72 hours of a child receiving emergency medical care was not available.

Correction Required: The operator shall immediately notify a child's parent and shall telephone notice to the appropriate regional office within 24 hours if a child in care at the facility is hospitalized or receives emergency room treatment. The operator shall mail or deliver a written report to the appropriate regional office within 72 hours after hospitalization of a child in care, emergency room treatment of a child in care, death of a child in care, a facility fire requiring the services of a fire department.

Noncompliance Area: The health reports of child #2 and #3 did not include statements that the child is able to participate in child care and appears to be free from contagious or communicable disease and a statement that age-appropriate screenings recommended by the American Academy of Pediatrics had been completed.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. A health report shall include a statement that age-appropriate screenings recommended by the American Academy of Pediatrics were conducted since the time of the previous health report required by this section.

Description: Child's hands washed/Adult Hygiene

Noncompliance Area: It was observed that staff person #2 did not wash their own hands and a child's hands after diapering. (corrected during inspection)

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Description: Disposable diapers

Noncompliance Area: It was observed that staff persons #2 and #3 opened the hands-free covered can with their hands when discarding a disposable diaper. (corrected during inspection).

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: It was observed that Staff person #1's medication was accessible to children in the classroom's emergency bag (corrected during inspection).

Noncompliance Area: It was observed that the water temperature of the classroom sinks in the Preschool and Young Toddler rooms exceeded the permitted 110 degrees F temperature (118 and 114 degrees F respectively)..

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F.

Description: No peeling paint or plaster

Noncompliance Area: It was observed that there was peeling paint in the Preschool, Discovery Preschool, and Young Toddler classrooms. (All corrected during inspection.)

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
