Findings: Upon review of the Emergency, Information, and Immunization Records of enrolled children at the facility, Surveyor 1 noted that the immunization record for Child 6 was not attached to their emergency card. The facility director attempted to locate the immunization record but was unable to do so.

Findings: Upon inspection of the one/two year old activity area and the two year old activity area, Surveyor 1 measured the water temperatures of the hand washing sinks in each activity area to be 84 degrees F. On July 22, 2020, the facility director submitted documentation to verify that the water temperature had been adjusted to be in between 86-110 degrees F.

Findings: Upon review of medications for enrolled children, Surveyor 2 noted that the authorization form for Child 7 's medication had an end date of 3/21/2020, however, the medication had not been returned to the child 's parents, nor the authorization form updated.
