| Findings: Lack of Supervision. Children were found entering a busy roadway. They had left the facility unnoticed by staff. Motorists stopped and assisted the children back to the facility with the assistance of law enforcement. Resolution: Provider has since put into place new policy and practices that ensure children are not able to leave the facility again. Provider is also working on additional barriers and notifications that would alert staff if a child has left the area where supervision and care is occurring. | |||
