The sponsor or sponsor representative, and each staff member shall complete a signed consent form provided by the Department that indicates the identifying information necessary to conduct a CARI background check, including the person’s name, address, date of birth, sex, race, and Social Security number. Pursuant to the Federal Privacy Act of 1974 (P.L. 93-579), the Department shall advise each such person that the disclosure of his or her Social Security number is voluntary, and that the Social Security number will only be used for the purpose of conducting a CARI background check.

As a condition of securing a license or Certificate of Life/Safety Approval, the sponsor or sponsor representative shall ensure that a Criminal History Record Information (CHRI) fingerprint background check is completed for himself or herself, and for all staff members at least 18 years of age who are or will be working at the center on a regularly- scheduled basis, to determine whether any such person has been convicted of a crime, as specified in P.L. 2000, c. 77 (N.J.S.A. 30:5B-6.10 to 6.17). The sponsor or sponsor representative and each staff member shall complete the electronic fingerprinting process through the vendor authorized by the State to conduct CHRI background checks through the Division of State Police in the Department of Law and Public Safety and the Federal Bureau of Investigation.

I have used this provider for less than 6 months

NOT CLEAN, BAD MANAGEMENT, PRICEY !!!!
I just put my baby there for 2 weeks ( 3 days in the first week and 2.5 days in the second week). First day, i picked my baby found her sleeping on the dirty carpet (not inside her crib where it supposed to) with her head down since she like to lay on her belly. First week, my baby got cold really bad. Apparently, they mixed my baby spoon with other kids who sick. I saw couple kids have runny nose pretty bad. They not even try to clean their nose. First day of second week, my baby got send home since she got foot, hand and mouth disease from that place. Apparently she got it since the first week since the doctor told me, it took 3-6 days before the rashes come out. She has fever the whole first week which I thought it was from her cold. With the doctor letter, I send her back to find that at the last day of second week, I found 2 bite marks on her arms. I confronted the teacher, she gave me running around not admitted any bite marks. I wonder how they watch the kids? I complained to the director, she even asked me whether my baby has that bite marks today. Not even apologize if its really happened or try to give me a solution or to comfort me. That's the final straw, I CAN NOT trust my baby to Kids Academy, Cherry Hills. This place is a nightmare for parents.
Pro: Love Mrs. Jazalin the morning infant teacher. She is the best.
Con:
- Not clean, they mixed your baby's stuff like bobby pillow, solid food and spoon.
- Bad management. When you have problem, you can't even talk to the director.
Mrs. Debra Simmons will give you running around and not take any action or apologize.
