Violation Observed: Ensure that the center notifies the OOL prior to relocating. The owner stated the center relocated the children (2/10/2020 - presently) to their location in South Orange due to construction at the Irvington site.

Violation Observed: The center shall ensure the children’s health, safety and general well-being at all times. On 2/18/20 the OOL arrived to conduct a reinspection and found the building locked with no staff and/or children present. The center illegally relocated their enrolled children to another currently operating program (also licensed by JaJo). The concern is that the building that the sponsor relocated to was violating the Certificate of Occupancy by having too many children present in the center and creating overcrowding conditions. Other violations/concerns included inadequate supervision and staff/child ratios, lack of activities, transportation in which violates MVC and DCF regulations, not meeting the needs of the children and not ensuring safe practices. The local health, fire and building officials were notified and responded to the concerns.

Violation Observed: Recited: Ensure that the children are directly supervised, wherein 18 children, ages 2.5-3 years old, were left unattended when a name stage would answer the front door multiple times. Also, a named staff was on the center phone and personal cell phone multiple times, while supervising a group of children.

For early childhood programs, the following shall apply for infants (under 18 months of age) and toddlers (18 months to 2½ years of age): if more than 12 infants or 20 toddlers occupy an individual room or a specific area within a large room, areas for up to 12 infants or 20 toddlers shall be defined by visible barriers, partitions, or any other room dividers or separations having a height above the eye level of the children who use those areas.

Violation Observed: Recited: Ensure that the children are directly supervised at all times, wherein staff were unaware that2 named children, 2.5-4 years of age, opened the door in Room 1 and went into Room 3 to use the bathroom. This occurred multiple times.

Violation Observed: Ensure that the children wash their hands with soap and running water after a diaper change in Room 2.

The center shall be permitted to obtain a valid fire safety inspection certificate issued by the municipality in which it is located, based on a fire inspection conducted within the preceding 12 months, and submit a copy of the certificate to the Office of Licensing in lieu of a CO or CCO, if the center serves only children 2 ½ years of age or older and is located in a public school building that is used as a public school.

Violation Observed: Ensure that the center does not serve more and 5 children under 2.5 years at a time, wherein children under 2.5 were present in Room 2 and a named staff member stated there were 17 children 2-2.5 years present in Room 1.
