Initial Comments: The following deficiencies were observed at the time of the annual compliance inspection conducted on 7/08/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. Two of two Fingerprint Clearance Cards reviewed were valid via the DPS website. The following was discussed, but not limited to: *Bleach cleaner and gloves must be located next to the diaper changing table. *Update the posted Fire Drill Log after each drill. *Toilet brushes must remain inaccessible to enrolled children. *Toilet paper must be in a dispensable container.
