Noncompliance Area: At time of inspection, it was observed that the outdoor play yard has a capacity that is smaller than the facility capacity. However, the facility did not have a daily schedule to show when children are accommodated to use the outdoor play yard for large muscle play activity.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. Weather permitting, children shall be taken out doors daily.

Noncompliance Area: During unannounced inspection on Wednesday, 03/29/23, cert rep observed facility out of ratio. Group 1 consist of 1 staff with 1 infant, 1 young toddler, and 3 older toddlers. Group 2 consist of 1 staff with 12 preschoolers.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). TIERED LIS: The required staff:child ratios must be maintained at all times. The legal entity must submit documentation in a format approved by DHS proving that the facility is maintaining the required staff:child ratios at all times. The legal entity will confirm continuing compliance by sending copies of the documentation to the -------- Region Office on a weekly basis for a period of thirty days on a DHS approved documentation format.

Noncompliance Area: During renewal inspection on Wednesday, 03/20/19, cert rep observed several first-aid kits throughout the facility failed to contain all seven items.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: During the renewal inspection on 4/6/18, the file for child #1 was inspected. Certification Representative looked at each paper in the file and found that the last health assessment on file was dated for 12/15/16.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: During the renewal inspection on 4/6/18, a hand washing sign with the requirement that an able child shall wash his hands after toileting and before eating was not posted by each sink in the facility.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: During the renewal inspection on 4/25/17, children were grouped in similar age groups in the facility; however, one of those groups were out of ratio because one staff person was caring for six children, the youngest in the staff persons group was one year old.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.
