Surfacing did not extend six (6) feet beyond the external limits of the equipment or 3 feet for equipment used only by children less than 2 years of age. The fall zone of the yellow and blue climber with slide and tunnel did not extend 6 feet in all directions as it was on the edge of the sand containment border and overlapped in the swing fall zone. The rock climbing wall was against the fence and it's fall zones were interrupted by the fence and the two-story wooden structure with the green slide.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. This information was not updated for staff members.
