| Findings: Ratio and supervision concerns. In room one of the facility there were 6 children less than 24 months old, and one child three years of age with a single provider, totaling 13 points per provider. The facility is in violation of Idaho State ratio requirements. Resolution: On August 6, 2019 a follow-up inspection determined that c hildren were properly supervised during the investigation. Healthy beginnings was within state ratio requirements. The inspector discussed ratio and supervision requirements with provider. | |||

| Findings: Investigation was in response to ratio requirements. Upon investigation, the child-staff ratio was not in compliant with city regulations. Resolution: At time of investigation the facility removed a child from one room and placed in another to meet ratio requirements. | |||

| Findings: Sleeping cots were broken, and wall needed to be repaired. Facility has several sleeping cots with damaged liners in the 5 year old room. Duct tape is being used to hold carpet and plastic covering to walls. Chipped paint and dry wall was observed in 3 year old room. Resolution: Sleeping cots were replaced, walls and carpet have been fixed. | |||
