A safe indoor and outdoor environment was not provided for the children. On the playground, rust is exposed on the bench near the stationary structure and rust is exposed on the steps of the structure. There is an exposed concrete area in front of the stationary structure. Next to the picnic tables there is a metal safety sign that is down. The lid to the toy storage container is broken. Plastic chip bags and other debris were located in the picnic table area.
