The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children.

Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. On the day of the inspection a child was found in the cafeteria without a staff person. The classroom had just walked out of the cafeteria to go to the bathroom and the child stayed in the room. The child was found by the inspector just after the last child left the classroom.

Violation Observed: Ensure that all staff have completed training pertaining to the center's policies and procedures for tracking children and are aware of the number of children in their care at all times. A named staff left the classroom with a group of children and a child was left in the classroom and the staff person did not adequately keep track of the children in his care.

Violation Observed: Ensure that adequate staff/child ratios are maintained at all times throughout the center. On the day of the inspection there were only 3 staff for 52 children when a 4th person was needed to maintain ratios. A supervisor came to the center at the same time that the inspection started and she remained on site for the day. Violation abated.

Space and room requirements are as follows: at no time shall a center allow more children in attendance than the licensed room capacity.

Violation Observed: Ensure that the center complies with the licensed capacity of the classrooms at all times throughout the center. Center is now using a different side of a divided cafeteria without OOL prior knowledge. The area previously approved had be divided and must be remeasured for accurate room capacity. There were 50 children occupying a space which measures for approximately for 20 children. Ensure the center does not over occupy approved classrooms.

Space and room requirements are as follows: all space and rooms within the center to be used by children shall be inspected and approved by the Office of Licensing prior to their use. In making its determination, the Office of Licensing shall consider whether the space is too far removed, remote, or isolated from other areas of the center to be used by children. rooms or areas of the center that are not Office of Licensing-approved for use by children shall be made inaccessible to children.

Violation Observed: Ensure that unapproved space is inaccessible to the children at all times. Center is using rooms that have not been approved by OOL. Center needs to request the new rooms for approval, and not use a new room without prior approval form OOL.

Violation Observed: Ensure the health, safety, and well-being of the children served. On the day of the inspection observation of the children in the gym indicated that two named children entered into a physical exchange and the staff in the gym did not notice until the inspector pointed it out to the staff. Ensure that staff are actively supervising the children.
