Plan: "Ratio policies were reiterated and clarified with all teachers. Teachers were reminded of protocols for notifying management on duty if a teacher needs to step out of the classroom for any reason. Management will respond quickly to all requests to help ensure DHS Ratios will be met at all times".
Description: Additional Non-Compliance Found During Investigation: Ratios- A staff member stepped out of the classroom, this left the classroom out of ratio.

Plan: "Prior to DHS notifying us of said complaint, Sunbeam learned of concerns that a staff member's interactions with another staff member did not demonstrate prudent and responsible behavior that ensured children's health and safety. Sunbeam investigated and swiftly terminated employment with the staff member concerned. The staff member was no longer employed with Sunbeam with DHS notified Sunbeam of their complaint."

Plan: "Added DHS reporting to Program Information sheet which is posted at the front desk. Oklahoma Human Services Licensing Requirements for Child Care Programs page 13 has been printed and placed behind the Program Information sheet to reference 340:110-3-280. Per the Program Information sheet, the following information is listed. Harbison EEC will notify DHS no later than the next business day of any unscheduled temporary/permanent program closures or relocation. We have added this information to our program information form which is posted at the front desk along with phone numbers of DHS and our licensing specialists. DHS REPORTING Gas odor/leak, Fire, Carbon Monoxide exposure, or Anything to do with utilities or building closures; A child injury requiring emergency medical attention; Any disease or illness requiring prompt local or Oklahoma State Department of Health (OSDH) notification; In addition to reporting any reason related to 340:110-3-280. Reporting (DHS guidelines) Contact Taylor, Jenna, or Oklahoma Department of Human Services within the same business day." See file cabinet.
