Center did not report closure of Head Start for the day due to flooding from a leak. Director stated the water softener leaked and flooded the hallway and one classroom. They closed for the day and it was not reported.Center also did not report to consultant that a DCS report was called in on a child that was had marks on him severe enough that parent took him to the ER/Dr. Parent states child did not have marks when he came. Center states nothing happened and it did not happen while the child was at the center. DCS is investigating.
