Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 1/15/2026 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please email the current violation-free fire inspection report and current gas inspection report to the Compliance Officer. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Emergency Disaster Plan update form was emailed at the time of the inspection. The following was discussed but not limited to: 1. Ensuring mats are maintained in good repair. 2. Ensuring the edges on floor rugs are not curled, posing a tripping hazard. 3. The cracked wall tile in the girls' toilet room. 4. All staff must document arrival and departure times. 5. Illness logs and what to document. 6. New staff training must be signed by the employee.

Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 1/21/2025. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Director at the time of the inspection. ***The Director designee was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 1/24/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 2 staff files reviewed. 2 of the 2 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Program Assessment was completed at the time of the inspection.

Initial Comments: Based on the Surveyor's observation, the parent posting board did not list the name of the current Assistant Director. The Director stated that the Assistant Director who was listed no longer works at this site.

Initial Comments: Based on the Surveyor's review of the licensee's documentation and a discussion with the Director, it was determined that the licensee lacked documention of a current gas inspection. The Gas inspection on file had expired on 2/20/2021.

Initial Comments: Based on the Surveyor's observation, the January 2022 menu posted on the Head start's program's posting board in room #41 stated "Yogurt", "Turkey tacos", "Lasagna roll ups" and " Juice" on various days of the month but did not state the specific components of these items.

Initial Comments: Based on the Surveyor's observation, one of the urinals in the hallway boy's bathroom was wrapped in black plastic and the Director acknowledged that the urinal was not working.
