Description: At the time of the visit, there was a large hole in the fence that encloses the outdoor play space. Also, A metal livestock panel was being used as part of the fence. The openings are 6 inches X 8 inches which exceeds the 4 inches maximum allowed.

Description: A 2 year-old child was injured when he walked in front of a 2 1/2 year-old child coming down a roller coaster ramp in a plastic car. The provider was within 5 feet of the roller coaster but not supervising close enough to prevent the injury. Also, the manufacture's recommendation for use of the roller coaster is for children 3 years and older.
