Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. At the time of the inspection a named staff in room 13 was not adequately supervising a child that resulted in the child's hand being pinched in the door while it was closing.

Violation Observed: Based on complaint 22345, Ensure that food provided by the center is stored, prepared and served in a sanitary manner. Food brought from outside of the center for a child shall be inspected by a staff member for spoilage before it is served. The center shall ensure that milk provided from the vendor is inspected prior to being served. The center took immediate corrective action this violation was cited and abated.

At the time of the inspection non ambulatory children were on the ground in bouncy seats on the playground, while ambulatory children pushed cars and were actively playing with children moving with ride on toys. Ensure the children's safety by providing an inactive space for non ambulatory children while on the playground.

Based on information, ensure that staff utilize positive methods of guidance and discipline. A named staff grabbed a named child's wrist area, dragging him from the playroom back to the classroom. At the time of the inspection positive interaction was observed. This violation was cited and abated.

Retrain all staff on the centers policy to ensure the children's health, safety and well being of non ambulatory children while on the playground.

For early childhood programs, the following shall apply, outdoor space requirements for children over the age of 10 months are as follows: for children in attendance for three or more consecutive hours, existing center(s) shall provide a minimum of 150 square feet of net outdoor space. When more than five children are using such a space at one time, there shall be 30 square feet of net outdoor space for each additional child in addition to the required minimum of 150 square feet. New and relocating centers licensed on or after September 1, 2013, shall provide a minimum of 350 square feet of net outdoor space. When more than 10 children are using such a space at one time, there shall be an additional 35 square feet of net outdoor space for each additional child in addition to the required minimum of 350 square feet.

Violation Observed: Ensure that the outdoor play area meets the required minimum square footage. At the time of the inspection there were 40 children on the first gated playground that has space for 32 Children at one time. Ensure the capacity of the playground is maintained at all time. Provide an MOU.
