Plan: I am going to take down the saucer swing and the rope swing and it will not be utilized until we find a safe and appropriate place for it. Prior to utilizing either piece of equipment I will verify with licensing that all equipment requirements are met. Director had the child on the equipment during the time of the visit stop using the equipment.

Licensing observed the out door area, which had two pieces of multi axis equipment that did not have a clearance space and surface under and around the piece of equipment onto which a child falling or exiting from the equipment would be expected to land. Licensing observed that under both of the multi axis equipment there was not a required use zone or impact absorbing material. Both the rope swing attached to the tree and the saucer swing attached to the had a fall height that was higher than four feet.

Plan: I will arrange to have new impact absorbing material dumped onto the playground and spread out under the fall zones and the required structures, 4 ft tall or higher. I will make sure to inspect the play ground and replenish the material as needed.

Licensing observed the out door area, which had two pieces of multi axis equipment. One was a rope swing attached to a tree, and the fall height was higher than four feet, and a saucer swing, also attached to a rope and to the tree with a fall height higher than four feet, both did not have impact-absorbing material, which are required in use zones.
