There were two personnel files reviewed. Both of the fingerprint clearance card were verified to be valid through the DPS website at the time of the inspection. Complete and submit a Plan of Corrections via the online portal within 10 days of receipt of this Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided to the facility at the time of inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider.

Initial Comments: There were no deficiencies found at the time of the initial monitoring inspection conducted on April 30, 2025, and are subject to changes pending programmatic review. Note: A full inspection was not conducted. Three of three fingerprint clearance cards were verified to be valid on the DPS website during the time of the inspection. The home was unable to be licensed at the time of the inspection.
