Children were not adequately supervised at all times. While reviewing of video from the day of the incident identified in this complaint, I observed a class of four pre-school aged children playing and then gathering to go inside the building from the playground area. While a group of three children are seen entering the building with the teacher, one child was observed playing in an area on the playground which has been identified as the sand pit area. The staff member enter the building without the child at approximately 5:00pm. At 5:05 and adult female is observed entering the classroom and appearing to search around the classroom space. She is observed walking toward the teacher and then immediately moving toward the exit door to the playground. The teacher and adult female are observed returning to the classroom with a child. The viewing of the cameras located on the playground shows the child running up to the adults.

Screen time was offered to children under three years of age. Children in a classroom with two years old were observed being offered screen time in transition from the playground.

Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. When children returned to the classroom from the playground, the staff member did not direct children to wash their hands.
