A staff member wrapped a child in their blanket, placed the child on their stomach while placing her hand on the child's head and rubbing their back in an effort to get the child to go to sleep. The child was crying loudly because of the staff member's actions.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Two electrical outlets not in use in space D110 were not covered. One electrical outlet, high on a wall in space E105, had exposed outlet hardware due to a missing faceplate.
