I have used this provider for more than 6 months

. 05/03/2018 At 20 months my child was held down on her stomach in a pack and play while she cried and screamed because she was having trouble taking a nap.it was brought to my attention by a staff member. At 2 my child was repeatedly left in a diaper for over 6 hours which resulted in a bleeding diaper rash. I marked the diapers so I know it was the same one. We are no longer at this day care and DHS has a report on this.
