Findings: A complaint investigation was performed on this date with Another agency and LS. The complaint alleges that caller states that her son was just looking at another child's laptop when the other child pushed her son into the table. Caller states her son's nose is swollen and the parent states she didn't receive an incident report or notice that this happened. Director informed LS that child was in classroom #1 with ET. R and another little boy was at table and little boy was on computer at table. R was standing next to boy P. P (age 9 or 10). R kept touching computer. P got upset and pushed R. R hit his nose on chair or table(not sure). R cried and that's when teacher asked what was going on. The teacher saw him on the floor, but didn't see what actually happened. Incident report was written but not complete. Director discussed that Ms. Ebony was in the classroom but didn't observe the actual fall. Director and teacher discussed with both children about the incident. No evidence of the incident report having all documentation regarding the fall. | |||
