Corrective Action: On September 30, 2019, the provider reported that several health observations were made and not documented as required. These concerns are to be retained in the child's file. This was discussed at the time of the visit and the provider agreed to comply.

Corrective Action: On September 17, 2019, a lack of supervision occurred when licensing specialists observed that an infant was sleeping in another area of the home and required 15-minute sleep checks were not conducted. No documentation of permission to sleep in another area of the home was on file. Ensure that written parent permission is obtained for a child under the age of two to sleep in another area of the home and ensure that 15-minute sleep checks are conducted. Submit parent permission to sleep in another area of the home and one week of infant daily logs showing the 15-minute sleep checks have been conducted.

Ensure all court orders obtained and policies documented in the parent handbook are followed at all times. This was corrected when the child was disenrolled on September 20, 2019.

Corrective Action: On September 17, 2019, the provider reported two children had started receiving care in her home on this date. No child files were present for these children. The provider was unsure of the last names of the children. Ensure child files contain all required documentation. Obtain a complete child file for two children including an infant feeding schedule and permission to sleep in another area of the home for Child #2. Submit all required documentation. The provider reported a child was enrolled for care in March 2019, and court documentation was obtain in July 2019, that stated one parent was not permitted within 100 yards of the family child care home. When this occurred, the child file was not updated to remove this parent as an authorized pick-up person. Ensure child files are updated when there are changes to the information provided by the parent. This was corrected when the child was disenrolled on September 20, 2019.
