Description: Fire safety - 1 yr.

Noncompliance Area: Staff #1 and #2 did not update their fire safety training annually as evidenced by a previous fire safety training dated 11/24/25 and a current dated 1/05/26 for staff #1 and a previous fire safety training dated 1/2/25 and a current dated 1/6/26 for staff #2.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: In the downstairs sink by the diaper changing area, the water temperature measured at 113 degrees. In the downstairs childcare area, the standalone sink's temperature measured at 111 degrees.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F.

Noncompliance Area: During unannounced monitoring conducted 8/28/2024, staff person #2's pediatric first-aid/CPR certificate expired on 4/14/2023 and was not renewed until 2/3/2024.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Description: Comply with CPSL/CPSL information

Noncompliance Area: During unannounced monitoring conducted 8/28/2024, FBI clearances on file for staff person #1 are dated 6/16/2019 and 6/21/2024, which is past 60 months.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Description: Health information

Noncompliance Area: Child #1 did not have an initial health assessment on file within 60 days following the first day of attendance. The health assessment on file was dated 9/23/23 (see code sheet for start date).

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Description: Child's hands washed/Adult Hygiene

Noncompliance Area: During the renewal inspection on 2/12/24, children and staff went to the table for lunch without washing hands.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Description: Exam communicable disease

Noncompliance Area: As of the renewal inspection on 2/12/24, the health assessment for staff #1 stated that there was a communicable disease with no further explanation.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination.

Description: Letter to parents

Noncompliance Area: As of the renewal inspection on 2/12/24, the letter provided to parents explaining the emergency procedures did not include information on lockdown, or accommodations for infants, toddlers, children with special needs or chronic medical conditions.

Correction Required: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan.

Noncompliance Area: As of the renewal inspection on 2/13/23, the emergency contact information for child #1 did not include the address of the physician.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: As of the renewal inspection on 2/13/23, the emergency contact information for child #1 did not include the address of the persons designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Description: Exemption documentation from parent/guardian

Noncompliance Area: As of the renewal inspection on 2/13/23, the health assessment for child #2 indicated that the child had not received any vaccines but there was no exemption letter on file from the child's parent.

Correction Required: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.
