Noncompliance Area: At the time of the inspection at least 7 of the 'nap mats' observed at the facility were torn and had exposed foam.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: At the time of the inspection no emergency contact forms of children in care or plan for transporting a child in the case of an emergency were located in the large muscle play spaces.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: At the time of the inspection peeling paint was observed in multiple locations of the preschool room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Potentially hazardous foods were observed in the lunch bags of children in the toddler classroom during the inspection on 6/27/18.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: At the time of inspection, the health insurance policy number was missing from the emergency contact forms facility children #1 and #2. Within six-months of the inspection on 6/27/18; the emergency contacts forms for children #2 and #3 were not updated .

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information

Noncompliance Area: At the time of inspection on 6/27/18, there was no written proof of a current health assessment on file at the facility for facility child #3.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: AT the time of inspection on 6/27/18, there was no written proof of liability insurance on file at the facility.

Correction Required: The legal entity shall have comprehensive general liability insurance to cover the persons who are on the premises. A current copy of the insurance policy shall be on file at the facility.
