In the bathroom located across from space #1 and labeled 'Pre-K,' an aerosol can of BA-FIX Germicidal Utility Cleaner was stored on top of the white wire shelf above the diaper changing table.

In space #3, the incident reports were not placed in the child's file. The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Verification that the EMC plan was reviewed with staff annually was not available for review for three (3) staff.

The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. Per Sanitation Rule .2817(a), Storage in toilet rooms shall be limited to toileting and diapering supplies. The bathroom across from space #1, labeled 'Pre-K,' had equipment used by children stored when you first walk in the bathroom and to the left of the diaper changing table.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The substitute caregiver in space #1, did not have the Emergency Information Form on file and the administrator/Principal did not have the Emergency Information Form on file in space #1 or space #3.

Six (6) gallon sized bottles of Clorox Bleach was stored in the unlocked storage closet in space #3.
