Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.

Noncompliance Area: In the Preschool 2 room, observed a children's round wooden table which had a leg coming loose causing the table to be unsturdy.

Noncompliance Area: In the infant room, observed child #3, an infant, sleeping on his back in the crib. The top part of the crib had a wedge underneath the mattress causing the child to sleep in an inclined position and not the sleeping position recommended by the American Academy of Pediatrics. There was not a documented medical reason on file.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: The most current emergency contact information on file for child #1 lacks the addresses of all individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The health report on file for 7 year old child #2 is not dated in accordance with the requirements for medical examinations for school attendance. Health report was completed on 8/30/2016 when the child was 3 years of age.

Noncompliance Area: In the infant room, observed two pack n plays and one crib which had loose fitting sheets causing a strangulation hazard and posing a threat to the health and safety of the children.

Noncompliance Area: On 1/14/2020, it was observed that there was not an operable fire detection system. Review of the Simplex Fire Detection panel showed trouble with eight of the interconnected smoke detectors. There were six on the second floor and two on the first floor which the panel detected as troubled. Conversation with maintenance person who stated that he disconnected the eight smoke detectors. This condition at the facility poses a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: In the preschool eating area, observed that the first aid kit was in reach of children.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: There was more than 24 months in between the previous health assessment and the most current and updated health assessment on file for staff #2 and #3.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter.
