A qualified staff member was not present. LS Kalafut reviewed the requirement for all children to be supervised at all times by a qualified staff member. Ms. Butler agreed to comply.

Corrective Action: Ensure the center is following all applicable federal, state, local laws, and regulations. Per the Governor’s COVID-19 State of Emergency, all school-age children are not allowed to enter the center without face masks and are required to wear face coverings throughout the day unless eating. At the visit, nine school-age children were observed in the school-age classroom without face masks or coverings. This was corrected when Ms. Butler provided face masks to the children in care.

LS Kalafut requested to see files for the three school-age children, including written parent permission to walk to the bus stop alone. Complete files were not present. A follow-up visit was conducted on September 16, 2020. At the time of this visit, LS Kalafut and LS Vandewater observed an incomplete file for one of the three children. Parent permission for all three school-age children to walk to the bus stop alone was observed. Submit complete files for three school-age children.

In addition the staff member used “time-out” for extended periods of time rather than one minute per the age of the child.
