Initial Comments: Based on facility documentation and the Surveyor's observation , it was determined that the file for Staff #1 lacked the following required documentation: Staff #1: The name of the individual to be notified in case of an emergency, the staff member's written statement attesting to current immunity against measles, rubella, diphtheria, mumps, and pertussis, a TB test result read on or before the staff member's starting date of employment (DOE: 12/01/2021, TB test result not documented), 10 day orientation training, and two good faith effort references from a previous employer. Initial Comments: Per allegation and based on staff interview, facility documentation and the Surveyor's observation, in the outdoor activity area, the climbing structure was not age-appropriate for 2-year-old children. The manufacturer's specifications stated the intended ages for use is 3-12. Staff stated that children as young as two-years-old are allowed on the climbing structure.

Initial Comments: Based on the Surveyor's observation, the following hazards were accessible to enrolled children: Restroom: *There was a blue container lid taped on the toilet tank in lieu of a toilet tank cover. *The gray baseboards located throughout the restroom were starting to pull away from the wall. Playground: *There was rough, broken wood located on the floor in the playhouse.

Initial Comments: Based on staff interview and the Surveyor's observation, the following was observed: Restroom: *There was a toilet that had a black trash bag located over the toilet seat. Staff stated that the toilet is currently not flushing. *There was a build-up of dirt and residue located around the plumbing pipes between the toilet seats and toilet tanks on two toilets. *There was a build-up of dirt and residue located around the edges of all sinks and around the faucet handles.

Initial Comments: Per allegation and based on the Surveyor's observation, the outdoor activity area lacked the following required resilient surfacing: Outdoor activity area: *2-4 of the required 6 inches of loose sand in the fall zones of the geo climber. *4 of the required 6 inches of loose sand in the fall zones of the metal slide. *3 of the required 6 inches of loose sand in the fall zones of the climbing steps.
