Initial Comments: There were no deficiencies found at the time of the Compliance inspection conducted on November 17, 2025, subject to changes pending programmatic review. A full inspection was conducted at this time. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The DES group size was observed to be compliant at the time of the inspection. The Emergency Disaster Contact form was emailed to the facility for completion. Items discussed, but not limited to, were: -New rules, -Storing items in the bathroom, -Renewing license via the portal - Anniversary Application in "Applications" or "Application History", -Renew license before 12/31/2025, -Review all staff files for expiring fingerprint cards.
