Violation: The facility's outdoor space was not safe/well-maintained for children's activities as evidenced by there was a large broken branch with sharp dried edges observed to the left of the back door, there is an area to the right of the back door near the fence with a pile of spiky balls/gumballs and dried branches observed. There was an uncovered metal fire pit observed on the concrete patio half way filled with water .
