Noncompliance Area: ON 12/9/19 DURING THE RENEWAL INSPECTION, THERE WAS NO DOCUMENTATION OF INFLUENZA OR HEPATITIS A VACCINE FOR CHILD #1, AND NO IMMUNIZATION RECORD ON FILE FOR CHILD #2.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: At the time of the renewal inspection, the file of child #1 had an emergency contact form but it was not signed or dated within the past 6 months.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information

Noncompliance Area: At the time of the renewal inspection, the file of staff #1 did not have a health assessment or TB screening completed within 12 months prior to providing initial service in the center.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: At the time of the renewal inspection, the file of staff #1 had a transcript from college in her file but the name was only written on the transcript making it invalid. The file did not have a copy of the high school diploma.

Correction Required: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: At the time of the renewal inspection, the file for staff #1 did not include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Noncompliance Area: At the time of the renewal inspection, there was no verification that staff had received emergency plan training on an annual basis.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: At the time of the renewal inspection, fire safety training for staff who have been working at the center for more than one year did not have verification of fire safety training in their files.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: At the time of the renewal inspection, the first aid kit in the PreK classroom did not contain scissors or tweezers.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: At the time of the renewal inspection, the emergency contact information form in the file of child #1 did not contain the health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: At the time of the renewal inspection, there was a bottle of medication in the infant room that did not have the original label clearly outlined on the bottle. There was also an expired medication in the Toddler classroom which was immediately disposed of.

Correction Required: Medication shall be stored in accordance with the manufacturer`s or health professional`s instructions on the original label.

Noncompliance Area: At the time of the renewal inspection, the TB screening on the health assessment of staff #2 was not verified with a date or indication that it had been completed at the time of the initial health assessment.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Noncompliance Area: At the time of the renewal inspection, there was evidence that the FBI clearance had been completed for staff #1 but the actual clearance verification showing that she was eligible to work in childcare was not in her file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1may not work in a child care position at the facility. A facility person`s record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: At the time of the renewal inspection, the file of staff #1 did not have verification of a high school diploma and diploma from the Chester County Technical School and therefore cannot be considered an assistant group supervisor until such verifications are in her file.

Correction Required: An assistant group supervisor shall have a high school diploma or a general education development certificate and have completed a post-secondary training curriculum described in 3270.31(c).A facility person`s record shall include verification of child care experience, education and training prior to service at the facility.
