Staff #1 had planned to be finished working at Creative Kids at the end of 2025. We did not require her to complete the fire safety training because she wasn't going to be working after the end of the year. When January 2026 came, staff #1 had a change with her plans and requested to continue working at Creative Kids. We were not able to go back to 2025 to have her complete her fire safety training, so a violation was given.

Staff #1 had started working at Creative Kids in November 2024....she had until February 2025 to completed her preservice training (CPR/First Aid, Health & Safety Courses, and Mandated Reporter Training) Those trainings would give her the required 12 hours of training. We did not require her to complete additional training in 2025 because we thought the preservice courses ,that were due by February 2025, would have counted towards the required hours. Staff #1 notified us that she wasn't planning to continue working with us after the end of the 2025 year, although in January 2026, staff #1 had a change with her plans and requested to continue working at Creative Kids. We were not able to go back to 2025 to have her complete any additional training courses, so a violation was given. Staff #1 began employment at Creative Kids on November 20, 2025. Prior to her "official" start date, staff #1 completed some preservice trainings, including Better Kid Care Health & Safety (10-hour course-completed 10/29/24), Mandated Reporter Training-(3-hour course-completed 10/3/24) and after her start date, she completed CPR/First Aid (7-hour course-completed 12/11/24). These trainings total 20 hours, exceeding the number of hours required. We were told by our inspection representative that the trainings Staff #1 completed in October 2024 would not be counted toward the 12 hours of yearly training requirements because they were completed prior to Staff #1's start date of November 20, 2024. The Better Kid Care,10-hour course, that was completed 22 days prior to being able to be counted, less than 1 month, along with her CPR/First Aid training, gave Staff #1 over the required hours (17 hours total) and should not have been a violation. Creative Kids disagrees with this violation, as the trainings were current, completed shortly before the hire date, met the required preservice training, as well as the required 12 hours of yearly training.

The siding on our building that was damaged was repaired with duct tape during the inspection on 2/17/26. Our students had not used the outdoor play area for several months due to the winter weather we had in 2025...(snow, ice, high winds...). We believe the siding and damaged gate pieces must have cracked and been damaged during the severe winter weather. The outside play area is always checked in the spring to be sure everything is safe and in good repair for our students to use.

The 11 foam blocks were in the a hallway storage cabinet not in a classroom that was used by toddlers. They were NOT in a classroom that was being used by toddlers. These blocks were not accessible to children who were still placing objects in their mouths. I'm being told that I must write a plan of correction, so we will continue to not allow any foam blocks, with apparant teeth marks, in classrooms that have toddlers or students who are still placing objects in their mouths. I've asked to have clarafication from my licensing representative through emails and phone calls, however, I have not received any response.

We knew that Child #1's parent, not the authorized pick-up adults, with the missing house number, was picking up on 6/24/24, Chid #1's first day, so we knew we would not need to verify the house number. The backpack for a student who needs medication is kept in his backpack in the classroom on a high shelf so it is out of the reach of children. We always, at least for the past few years have kept anything that is required to be out of the reach of children, including the classroom first aid kit on this shelf and have not received a violation. This year were were told it was not a "high enough shelf" due to us having taller students, I guess, in this room (4-5 year olds). The backpack containing the medication was moved from the 3rd to the 4th shelf when our certification representative was still in our building.

The classroom walls were recently painted, so it seems as though when everything was taken off the walls, the daily activities must not have been put back up. During the time certification staff were still at the school, the daily activities document was located and put back up in room #2.

Staff #3 and I thought that she had completed the Health and Safety one hour update training by 12/30/2022, however, when I conducted my review of her file in May 2023, to prepare for our yearly inspection, her certificate was not there. She logged onto her account on the Better Kid Care site and completed her one-hour training update, printed out her certificate of completion, and her certificate was placed in her file on 5/19/2023. ~SHAKEN BABY SYNDROME-¿MALTREATMENT¿: Last year in June 2022, at the time of the inspection, my Shaken Baby Syndrome Policy was reviewed and approved by my certification representative, and her supervisor. This year, my policy was NOT approved because now, I'm being told that my Shaken Baby, Abusive Head Trauma, and Child Maltreatment policy lacked the prevention and identification of Child Maltreatment. I sent my updated policy to my certification representatives with the following sentence added to my policy so that it can be approved again this year. "Staff are trained on prevention and identification of maltreatment and the causes/effects associated with that." ~ EMERGENCY PLAN-CONTINUITY OPERATIONS: Last year in June 2022, at the time of the inspection, my Emergency Plan was reviewed and approved by my certification representative and her supervisor. Since being told, and being given a violation last year, (June 2022) that we are no longer allowed to allow parents to put an icepack in their child¿s lunchbox to keep their child¿s potentially hazardous food items cold until lunchtime, even though we¿ve been doing that since 2009 when we opened, we have notified our parents that we are required to place their child¿s potentially hazardous food items in our refrigerators, due to our regulations. We ask the parents to hand any food items that need to be refrigerated to our teachers when they drop their child off, however, sometimes, especially on a busy morning, they can forget to hand their child's food items to a teacher, so the items may not get in our refrigerator immediately. Our wonderful teachers go through each child¿s lunchbox and take any items that may have been forgotten to be given to us by a child¿s parents, label the item(s) with the child¿s name, and place it in our refrigerator. We are extremely careful not to put children¿s food items together that could potentially be something another child may have an allergy to....(sometimes a very severe allergy). On June 8, 2023, child #3¿s yogurt was missed by both her parent and our teachers. One factor to note is that child #3 had a difficult drop-off, and was having a lot of trouble leaving her mom that morning. The teachers had to carry her in and provide comfort to her to help her to settle down, so they did not have a chance to go through her lunch box right away. I was very happy to be informed by my teachers that Ms. Mento and Ms. Pannetta went through ALL our students¿ lunchboxes during their inspection and only found one yogurt, that was missed because the child was being comforted and cared for by her teachers. Luckily, there were two icepacks in Child #3¿s lunchbox, so the yogurt was cold. The yogurt would most likely have been found once child #3 became involved in an activity and one of the teachers was able to examine her lunch box food items. We will continue, as we¿ve been doing since receiving our violation last year in June 2022, to look for items that are potentially hazardous and put them directly in our refrigerator.

AFTER A FEW DAYS, IN A PHONE CALL WITH MS. THE SECOND OPTION I COULD TRY WAS TO ASK MY EMPLOYEES GO TO A NOTORY, MAKE A STATEMENT THAT THEY COMPLETED AT LEAST AN 8TH GRADE EDUCATION, HAVE THEIR STATEMENT NOTORIZED, AND THAT WOULD BE ACCEPTED. THE STATEMENT MUST BE DETAILED WITH THE NAME OF THE EMPLOYEES, THE REASON THEY COULD NOT GET THE INFORMATION, EVEN THOUGH THEY ALREADY HAVE THE INFORMATION, AND THEIR ATTEMPTS MADE TO ACQUIRE THE INFORMATION. HOWEVER, IF THEY DID OPTION #2, THE DHS WOULD ACCEPT THEIR NOTORIZED STATEMENTS AND I WILL BE ABLE TO RECEIVE MY CERTIFICATE OF COMPLIANCE TO CONTINUE TO OPERAT MY PRESCHOOL CENTER, HOWEVER, MY EMPLOYEES WOULD ONLY QUALIFY AS AIDES, EVEN THOUGH THEY BOTH ARE QUALIFIED TO BE ASSISTANT GROUP SUPERVISORS. WE WENT THROUGH THE PROCESS OF WORKING WITH AN "APPROVED MEMBER" OF THE NATIONAL ASSOCIATION OF CREDENTIAL EVALUATION SERVICES (NACES) OR THE ASSOCIATION OF INTERNATIONAL CREDENTIAL EVALUATORS (AICE). FINALLY, WE RECEIVED THE DOCUMENTATION FROM THE APPROVED AGENCY. WE RECEIVED THE SAME INFORMATION WE ALREADY HAD IN STAFF #1 & STAFF #2'S FILES. THEY BOTH ARE CONFIRMED AS COMPLETING AT LEAST A HIGH SCHOOL EDUCATION AND THEREFORE ARE BOTH QUALIFIED TO BE ASSISTANT GROUP SUPERVISORS AT CREATIVE KIDS EDUCATION CENTER. I EMAILED THE DOCUMENTATION FOR STAFF #1 & #2 TO JESSICA MENTO AND JANETTA PANNELL ON 7/3/23. WE'RE HOPEFUL THAT MS. PANNELL OR MS. At the time of enrollment, we inform our parents that they are required to either put an ice-pack in their child¿s insulated lunch box or if they do not have an ice pack, they are instructed to tell the teachers their child¿s potentially hazardous food items, such as yogurt, need to be placed in the school refrigerator until lunch time. Teachers check lunches each morning to be sure items that potentially hazardous food items either have an ice-pack or are placed in the refrigerator. I was told on 6/22/22, that ice-packs are not permitted to be used to keep food items cold in a child¿s lunchbox. It was suggested to me by the supervisor, that I have parents pack their child¿s lunch in a brown bag, so that we can put the entire lunch bag in the refrigerator, although I don¿t currently have enough space at my school. Staff #2 hours are complete at this time, because, due to very confusing regulations that were suspended and then reinstated, and the poor communication from the Department of Human Services, we were not aware that Staff #2 needed to complete the 12 hours from December 19 2020 through December 19, 2021. Then, we were given incorrect information for the date the required health and safety training to be re-taken, so, even though staff #2 wasn¿t required, she completed the additional training hours, and those 10 additional hours will count towards the 7 hours she needed. I was informed that since Staff #2 could use her 10 hour course to satisfy the 7 hours she needed to have completed by 12/19/21.

We now have 3 ¿wash your hands¿ signs (one over the sink, one over the toilet, and finally, one over the changing table.
