Country Kidz Daycare - Townville PA Child Care Center

Noncompliance Area: In the large child care space in the rear of the building, in areas accessible to children in care, two approximately 6" by 18" plastic bins were observed to have cracks with jagged edges that could cause laceration injuries. The plastic mounts of the gate separating the front child care space from the kitchen, in an area accessible to children in care, were observed to have jagged edges as well.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: A small window cling measuring less than 1" in diameter was observed on a window in the child care space near the facility's entry. The cling was peeling from the window, which presented the possibility that it would fall onto the floor in an area that is used by children who are still placing things in their mouths.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The file for Child #5 contained an emergency contact form that did not include the listed physician's telephone number.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: The file for Child #2 contained an emergency contact form that did not included signed/written consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: The file for Child #1, who is a young toddler, did not contain a health assessment completed in the past six months.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: The file for Child #4, who is approximately two-years-six-months old, contained an immunization n record that did not include the following vaccines based upon the recommendations of the ACIP: Two Hep B doses, three PCV13 doses, one Hep A dose, and an annual flu vaccine. The file for Child #3 did not include documentation of an annual flu vaccine in the past 12 months..

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: Staff #2 was observed diapering an infant. Following the diaper change, Staff #2 did not her hands, nor did she wash the hands of the child.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Noncompliance Area: The files for Staff #5 and #6 contained current health assessments; however, the previous assessments were completed more than 24 months ago.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file for Staff #2 contained a health assessment that was signed by a Certified Medical Assistant.

Correction Required: A health assessment shall be conducted and a report shall be written and signed by a physician, physician's assistant or CRNP. The signature must include the individual's professional title.

Noncompliance Area: The file for Staff #5 contained a current health assessment; however, said assessment did not include the results of an examination for communicable diseases; nor did it include a physician's assessment of suitability to provide child care.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: The files for Staff #1-#7, all of whom have been employed for more than 12 months, did not contain documentation of the completion of fire safety training in the past year.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: In areas accessible to children under the age of six, in the large child care space and infant rooms respectively, an unused, unprotected electrical outlets were observed.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Observed in the large child care space to the rear of the building on a countertop that is accessible to children in care, were two aerosol cans of Lysol and a container of bleach wipes, each labeled Keep out of Reach of Children

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: In areas accessible to children in care, in the large child care space, as well as in the front child care space, pressed wood baseboards were observed to have become unfastened from the wall which caused the brad nails used to secure the baseboard to become exposed. Contact with the loosened baseboards could cause puncture and/or pinching injuries. Also in an area accessible to children in care, the large child care space, a trim piece on the edge of a countertop was observed to be cracked with a jagged edge that could cause lacerations injuries. In the Doodlebug room, in an area accessible to children in care, a protruding brad nails was observed on the window trim.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In an area accessible to children care near the floor on the doorway to a hallway storage area chipping plaster was observed.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
