A container of 8 packs of Airwick Stickup air fresheners were stored on a shelf below five feet in the resource room.

All electrical appliances were not used in accordance with the manufacturers instruction. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. A laminator with a heating element and a container of 3 hot glue guns were stored on a self of the resource room, below five feet.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Authorization for prescription Nystatin was documented on the topical ointment form and did not include the medical condition or allergic reactions, frequency of dose, and up to six months authorization.

A violation was found for which there is no item number. During a diaper change in space #6, the teacher did not clean the child's hands with a wipe after the soiled diaper was removed. The teacher did not use a wipe used to clean her hands prior to putting on the clean diaper. This is a violation of Sanitation Rule 15A NCAC 18A .2819 (d) Diapering and Diaper Changing Facilities.

Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. In space #1, an albuterol inhaler was stored without the original packaging.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One of three staff files reviewed did not have a First Aid certification from an approved training organization. Two of three staff files reviewed did not have First Aid certification on file.
