Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. An approved fire inspection report from the April 22, 2025, was received during the visit.

Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Staff employed on June 17, 1996, had a qualification letter that expired on September 31, 2025. A new qualification letter was issued on September 2, 2025.

Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Staff employed 6/17/96, 6/17/96, 10/29/01, 2/10/20, 2/3/20, and 6/26/23 had First Aid certification that expired on August 31, 2025 and completed First Aid training on September 25, 2025, therefore they did not complete certification before the expiration of the previous certification.
